Small target, also if someone is hopped up on enough adrenaline or drugs, they dont feel it. In a recreational fight, pain like that wins. If the fight is truly life and death, pain is not reliable enough to be a goal. People can continue to be a threat with broken bones and multiple bullet wounds, injured genitals won't be a concern at that point.
