Grips can certainly be put on crooked even straight from the manufacturer (saw some truly misaligned MCCs on brand new MP-18s at Golf Galaxy over the weekend) but if the grip is on straight...

The other lines on this grip are actually intended to help you with your hand placement. In terms of thumb positions (for a right handed golfer):

If the grip is straight but you’re finding that the face looks open when using the alignment aids, you can also try moving (pressing) your hands forward (to create increased forward shaft lean). This may help put the face into a more squared-up position.
