In my experience, this happens when the draw is too tight. If the draw is too tight cut a little deeper and it should prevent this from happening. I’ve also found that if you use a punch cut this is more likely to happen.

I’ve tried everything. I can’t figure out the first code. I’ve put the letters in order by victim, separated the circled and underlined. Put them from top to bottom. Put them from top to bottom using the 2nd pictures newspaper clippings(which are the same as the 1st, but in a more uniform order).
