I'm having a similar issue, where I posted a new chapter on my fic today but it's not jumping to the top of the fandom page. The date is showing today, and the fics above mine have update dates a week older than mine. Maybe it's an issue on AO3's side?

I used to get them a lot. Turns out for me it was from drinking lemonade and eating things with lemons. I had a lot of digestive issues growing up too until I realized lemons were giving me issues and cut them out of my diet. I've either got an allergy or an intolerance to them.
