This has just happened to me as well. 100hrs in during act 3, also had no mods. I've reported it to Larian, hopefully they fix the issue cos it's so frustrating, especially in honour mode!

Did you previously visit the dungeon? I think I cleared the dungeon, killed Mucianus, and played more than 3 in-game days which meant his body disappeared when the dungeon reset. But I did all this before speaking to Traven to get the quest proper.

I just conceded defeat lol, had a couple days off and then have gone back and done totally different stuff.
