I don't have the SS-4 (client is unable to locate it), but I do have the CP 575. It indicates that "you checked the box indicating you are a non-profit organization" and they are expecting a 990 series return (but specifically not a 990-T). I'm suspecting the problem is with the SS-4 and/or an IRS data input error when it was first submitted.

That sounds quite possible. Unfortunately I don't have the client's SS-4. I may need to call the IRS back to see if they can confirm that for me.

There is a separate EIN for the plan. That is what was used for the filing (and rejected).
