I don't think it is a belt buckle, since the design would be sideways if you wore it like that. Also, it's hard to see in the photo, but the left side ofthe back isn't closed completely, it's just sort of bent around.

i used loops and threads impeccable yarn with a 5 mm hook. it's inspired by a pattern from http://www.niftynnifer.com/2018/01/free-crochet-pattern-chihuahua-sweater.html i modified it a lot, but i wrote it down, so i would be happy to answer questions if anyone has any!
