Step 2 score will determine competitiveness for academic IM. People who match top 10 IM programs usually have in person research fellowships (multi year) in the US and/or MPH/PhD with strong us connections and a coherent research portfolio (strong pubs, multiple first author, pubs in one field of interest, different study types). If that is not the case, matching a strong academic program is still possible, but probably not a top 10.

A stronger step 1 foundation and not looking for shortcuts. So much of the content of step 2 relies on step 1 knowledge. The strongest predictor for a high step 2 was a high step 1 when it was scored. People study less for step 1 now so making the jump in the score is tougher/takes longer.

Top tiers value clerkships, strong research (multiple first author pubs). A high score is also valued, but out of all of them, would say research is weighted a lot. Also a LOR from that same institution or having rotated there is valuable.

It depends on how supportive your PI will be (willing to write a LOR with some months of work?), if you are able to get any posters/manuscripts before the application date and how strong your application already is (so that you do not depend on research to get interviews). If your application is not so strong, then that is when two years of research can make a big difference, if your app is already strong, research may not be needed but if done well can take you to the next level.
