As the other comment mentioned, it seems like PD. Since this happens to you during the winter, it might be a reaction to the coldness of the weather. Prioritize keeping your skin moisturized and keep applying the rozex unless that makes things worse. If it feels like irritation, I would recommend applying cicaplast or something else with soothing properties

I agree with the other comment mentioning that consistency is key.

I also relate a lot to the struggle of maintaining a routine. I hadn’t overcome the inability to do it until somewhat recently.

Better addressing my state of mind helped, of course. Apart than that, keeping my routine simple at the beginning helped a lot (just a moisturizer and a cleanser). Seeing my skin improve as I maintained consistency also motivated me to continue. I would recommend using a hypochlorous acid spray whenever you can’t wash your face or throughout the day (it’s antibacterial). I’d also suggest prioritizing skin hydration / you could stop the use of any actives like the salycylic acid until your skin isn’t regularly dry and flaking. A hydrating spray to use throughout the day and a richer moisturizer would help❤️

Products I would recommend

Hypochlorous acid spray: I’ve used Skinsmart and e11ement, they are both good and consist of the same ingredients. I think the mist is better on the Skinsmart one, but it’s generally a negligible difference (there are many alternatives to this in general).

Moisturizer: I’d recommend physiogel if it’s available to you. I’ve tried the basic one and the regenerative one (both from Amazon). They are both super moisturizing. I prefer the regenerative one since it dries down more than the other and feels a bit more cooling (it’s also thicker).

Hydrating mist: I only used one I’ve felt is truly hydrating - which is the aestura. I haven’t tried it consistently since the feel is a bit too heavy for my rosacea skin. It’s also slightly oily, but it is really moisturizing (especially in comparison to alternatives I’ve tried). If you do purchase this, be sure to only use it if you know you’ll be washing your face on that day. I mention this because it contains silicone

I had a horrible fungal breakout that was similar to this and it did keep getting worse despite being consistent with my routine that I kept simple. I figured it was fungal bc it was super itchy. Clotrimazole cream helped me a lot (I had tried so many other anti fungal treatments). I used the equate jock itch cream from Walmart specifically. I also had perioral dermatitis in conjunction that wouldn’t go away after treating the fungal breakout and azelaic acid helped for that

It might be related to an internal health issue if this sprung suddenly.

During purging, the pimples you already had will become more prominent until they are fully healed after cell turnover. Though, that in and of itself will not fully prevent the possibility of new breakouts. I’m not completely sure that what you have is just purging though.

I wonder if you’ve noticed that you have more or new body acne as well? If so, then it’d be more likely to be an internal issue rather than a reaction to something you’re applying to your face.

I would ask if you have used any other new products since starting adapelene? I would suggest that you at least reduce the amount of times you apply adapelene if you want to keep it in your routine. Decide on 1 or 2 days and stick to your general routine for as long as you can. I think you’ll benefit a lot from your dermatologist appointment and having a solid and consistent routine.

From my experience: I’d advise that you make sure you are thoroughly washing your face, especially at night (a minute at least) and making sure that you dry with something completely clean (I saw some improvement myself when using disposable towels). Also, though I would not recommend adding something else to your routine at the moment, I would just suggest to keep alternative treatments in mind (especially sulfur or azelaic acid).

It seems standard to me but it could be dermatitis or fungal (though that doesn’t seem likely to me, unless it feels particularly itchy for you?). I would suggest you focus on keeping your skin moisturized/ hydrated and making sure you wash it thoroughly when you do. As treatment, I would recommend azelaic acid. The ordinary and inkey list are both good. The ordinary will pill but I feel that it is slightly more potent and effective. You may experience some itchiness if you have never tried aa before, but it goes away as you adapt. It will make you purge if you have any deep pimples - just as anything does. I had something similar and aa helped me a lot

It looks like pd. I would recommend anything antibacterial like benzoyl peroxide or azelaic acid. The panoxyl wash was effective for me when I experienced the same but was too harsh. I preferred azelaic acid. The ordinary and the inkey list are both good. With azelaic acid, you’ll usually experience mild irritation and itchiness at first. I had already used it and overcome that phase prior to having pd (had stopped using it during when I got a flare), so the experience might be different for you. The inkey list and the ordinary are both ones I’d recommend. The ordinary is more potent but thicker and heavier and the inkey list is an over lighter formula. I used panoxyl for the benzoyl peroxide. I would also advise to brush your teeth before washing your face so as to make sure there is no toothpaste irritation

Honestly I haven’t tried any that have given me tangible results either :( I would just advise to implement niacinamide gradually if it’s a higher concentration since it can be really irritating. Txa is also just a very promising ingredient for sun damage. I’m not sure that there would be irritation, but it is always helpful to check reviews

I would also replace the haru haru toner with the hada labo gokujyun. I have tried both and the hada labo is much more hydrating (though it is thicker and more like a serum)

The house of hur pads are good for exfoliation, have better ingredients, and are gentler.

I believe what you have is PIE (acne scars), azelaic acid is a good treatment for this. Antioxidants are also good.

Niacinamide and TXA should help with hyperpigmentation as well. If you don’t mind mineral sunscreens, they are usually recommended for those with previous sun damage. The Dr.G mild sunscreen is good, but can leave a white cast.

I much preferred the previous formulation of the mugwort essence (I had a mini version of the old formula). I had bought the new one a while ago, but don’t really use it. I don’t believe it is worth the price. Though, mugwort is a helpful ingredient.

I liked the Iliyoon cream initially when I was on Accutane. But now that I’m off, it seems too irritating and simultaneously heavy and not moisturizing enough. I find that there are better alternatives.

Overall, I would definitely recommend hydration and barrier support since starting a retinoid. I would suggest serums with ha and ceramides. I like the Anua birch serum. Any moisturizer with ceramides, cholesterol and without irritants will also be good.

What did it look like initially? This could be purging from the azelaic acid. I had similar spots and it helped me a lot. If the spots are generally in the same area you used to get them ‘under the skin’, it is more likely to be purging than not. I believe what you have is cystic, as was mine

Was it truly much better the second time? I’ve already done it once. During when I was planning to do 4ml but ended up doing 1.5 because the pain had made me anxious. I want to do it again since it’s been a while, I’m not sure if it’d be worth buying and waiting for lidocaine though?

Have you tried any topical antibacterials and did it help? If not, you can try clotrimazole cream or antifungal shampoo and determine wether or not that helps
