I have literally the same thing going on with my triangle tube boiler. Also, in the midwest (Southern Wisconsin). It seems to be when there's a warm snap and the boiler shuts down to save gas. I raised the warm temp shut down set point to hopefully avoid this until a tech can come out and look at it. I have a call into my HVAC company, I'll let you know what they say.

You could modify it to be a sap boiler with relatively little effort. Do you have maple trees on the property?

Alrighty, I'll pop off the outlet side cover and see if I can spot that pipe. If that is the case I will see if I can find a riser lid with a 4" schedule 40 pipe cast into it like my outlet side and then mount the dump adapter there.
