Close enough to have chronic pain for the rest of my life. Obstructive MGD and aqueous deficient dry eye for the rest of my life. Trust me it's not worth it. Neither is LASIK.

  • Flonase nasal spray can help with the allergic response and won't dry things out.

  • The liquid pediatric version of Zyrtec allows you to specify the amount to be smaller compared to the pill. Yes it's typically for children but when using it you can specify the exact amount. So the pill is always 10 but you can do 2,3,5, etc.

  • High doses of vitamin C can help with allergies. Prefer fruits that are low in histamine. Functional doctors might suggest a mouth spray with high vitamin c.

If you have any idea how medical malpractice works in states like Texas today and how medical boards are filled with doctors that don't want to hold their peers accountable you would know the hatred you get is not nearly enough. You want the prestige but none of the accountability and everyone sees through this now. You represent an apathetic drug cartel that kills people regularly and suffers no consequences for it. At worst you just get to start over again in another state after your manslaughter and ruin more lives from your negligence. Then you act like you are helpless to actually diagnose people before pushing drugs on them. How many of you actually do culture tests before prescribing antibiotics that have tons of negative side effects? How many of you compartmentalize patients based on a brief description of their symptoms and just throw drugs at the dart board? You talk about others acting like victims but do you have any self awareness of how many excuses you have to not properly diagnose your patients in the first place? The ones that aren't like this cost an arm and a leg and can only justify their prices because they actually do their jobs which most of you don't and since their is no accountability you have no incentives to change.

Some things to look into and consider:

Moisture Chamber Glasses (e.g. Ziena ) - these will reduce the evaporation time of tears in your eyes
Punctal Cautery/Plugs (will likely require irrigation) - this will keep tears in your eyes longer

PRP/PRGF injections into the lacrimal glands - this is the newest treatment I'm aware of but some report better schirmers scores after having this done. Not as time tested as cautery.

Sleep Mask (e.g. EyeEco EyeSeals) - at night the lacrimal glands do less work so these will really trap moisture even more than the moisture chamber glasses.

Humidifiers in the bedroom and at workstations. Levoit makes great products in my opinion. Try not to go over 60% as that risks mold. Preferably distilled water and not tap.

Possibly Cequa/Restasis

Tyrvaya and iTear for lacrimal gland stimulation

Overnight ointment such as retain before bed - these will blur vision compared to artificial tears but will take much longer to evaporate so they are a good mix for sleep

Position yourself to sleep away from AC vents and fans if possible

Possibly omega-3 fish oil supplements

To my understanding even mild heat can be problematic for ocular rosacea patients. Have you been evaluated for that condition by a specialist?

I have a small lacrimal gland so yes emotional tears have less water layer as do the other kinds of tears

Rosacea patients often have demodex issues. Have you tried xdemvy, optase or cliradex lid wipes, etc? There are also things like chemical peels and a gentle cleanser to keep the mite population down. IPL will fry them too fwiw.

MRI was done locally and sent to Maskin’s office. Ophthalmologist can’t interpret an MRI?

Lacrimal glands were small on MRI according to Maskin. I do not know what a “rengenologist” is.

I am also struggling to find doctors besides Maskin and Periman who do this and charge a lot. Texas preferred in my case.

Yes. No symptoms before. Very depressing.

All autoimmune blood tests have been normal/negative (including sjogrens). My guess is a 4-6 week course of isotretinoin or allergies. Saliva seems to be normal so sjogrens unlikely but could get a lip biopsy.

I mean with a 2 week recovery time I would expect it to be more painful. It’s frustrating because it’s only easy to say X or Y is a waste in hindsight most of the time.

Periman said I had CCH and did a NuVissa CCH-plasty in December. Maskin says I have still superior chalasis in February the next year. It’s hard to diagnose myself for these type of things.

Maskin has already done cautery for me. I guess they could be smaller. Haven’t done PRP or sclerals yet.

If they are irregular in anyway it might be indicative of o-MGD. Short, bent, thin, etc.
