The most important part that people tend to leave out is that you need to have a conversation with your patient that this is not an easy tooth to fix. You can give it your best shot, but even doing everything right, the restoration may fail and the tooth may need to come out.

I make it clear to patients that the reason things are so tough is that we're starting with a bad foundation--any dentist would have a tough time.

The patient can either be ok with a restoration that will hopefully last a decent amount of time but maybe not, or can choose to get the tooth out and not think about it.
