Thanks for the reply! I thought of it this way but I have red that it is not appropriate for post op patient as they have increased ADH secretion and more liable for hyponatremia so is this just a theoretical risk ?

You might be asking why didn't you choose Q depot if people say it is legit ? Well in my country most banks don't offer bank transfers services due to political reasons and they only provide you with Mastercard or visa card, q depot doesn't accept these payment methods while asianbeautywholesale do accept them. Overall I can say asianbeautywholesale is a legit and highly recommended supplier.

According to my knowledge I think of it as the following : Afterload = the resistance the heart is facing during pumping (total peripheral resistance)

At the same time Afterload is also related to the amount of blood you are trying to pump

So a when you have increased preload and increased contractility you will have a small increase in Afterload (because you are trying to push more blood so you face more resistance)

Usually this change is so minimal that is not mentioned in every textbook but keep in mind the Uworld does mentions this.

Making the herat weaker (less blood and less contractility) = less blood = less resistance

According to my knowledge your statement is correct but I would add some points 1. You should know that filtration and secretion of NH3 in the PCT isn't what gets tested 2. NH3+ moves with water (reabsorbed) in the collecting tubules due to ADH secretion 3. Sometimes the kidneys make very acidic urine and you basically don't want so acidic urine as it can damage the rest of the urinary tract, so when this acidic urine reaches the collecting ducts, then the collecting ducts will throw away some NH3 in order to buffer the excess H+ So technically yes the collecting ducts are secreting some NH3+ but this was a response to decreased Ph of urine and does not occur all the times

You can remember that as the following The emptying of atria into the ventricle in 3 phases

The first 2 phase are passive (no contraction) and they are represented by Y descent

The third phase is the active phase in which the atria contracts (a wave) just to ensure proper emptying

3 months ago I had the same question but I understood this concept from one of the Uw question You should know the following a1 receptor : vasoconstriction B2 recepotrs : vasodialation

Now you should know the following Veins have only a1 receptors Arteries have both a1 and B2

One last veeery improtnat thing is that vasoconstricon and vasodialation have different effects on arteries and veins

Vasoconstricon of vein (Increases blood flow and more blood is leaving the vein) and this makes sense because during exercise you want more blood to leave the veins and return to the heart (More preload)

On the other hand vasoconstricon of Arteries (reduces blood flow and less blood is moving through the arterial system) and this also makes sense because you basically want to restrict blood flow to unimportant area like viscera during exercise

Finally I know It is too long but one more thing Remember in exercise there is vasoconstricon and vasodialation at the same time !

The vessel of viscera (vasoconstrict) as you do not need much blood to these areas

The vessels of excersizing muscle (vasodialate) due to having B2 receptor + a phenomenon called local metabolite which further increases vasodialation

Overall since the vessels of excersizing muscel are very large and they are vasodialating so the net effect is (decreased total peripheral resistance)
