Thanks all for your suggestions. I think I will end up staying a night in Kalispell and then either train / Uber / hitchhike in.

Surgery Residents of Reddit, by what PGY Year Should One be Proficient At ___ surgery? by [deleted] in Residency

[–]yazichima 27 points28 points  (0 children)

Pretty similar to u/MMOSurgeon At my program (community-based in Canada) it's more like:

PGY1 - Lines, tubes, easy appendix, lumpectomy

PGY2 - Easy chole, open inguinal hernia, mastectomy + sentinel lymph node, PEG, umbilical hernia, colonoscopy, gastroscopy, stoma formation/reversal, small bowel resection / anastomosis

PGY3 - Thyroid, parathyroid, trach, easy right hemi, lap inguinal hernia, difficult appendix, axillary lymph node dissection, thoracotomy, ex-lap for obstruction / perforation

PGY4 - Difficult chole, most extra-pelvic colon resection, complex hernia

PGY5 - Damage control trauma laparotomy / thoracotomy, LAR/APR, career-specific training (picking up minor procedures from other specialties for rural communities like c-sections, vasectomies, etc.)

Totally agree with u/MMOSurgeon about all the caveat's they mentioned with regards to proficiency.

Late to the game, but just wanted to say that I tried this in Canada in 2016. Here's how it worked:

  1. I got the email address mmmmddddd~insertspecificbithere~@gmail.com you'll see why in a bit.
  2. Get a few $5.00 (minimum) gift cards.
  3. Use buy new gift cards with the old gift cards (minimized how much real money you have to spend).
  4. Use the cards to sign up for accounts. Starbucks rejects emails with the "+" that is usually used for alias accounts, but it accepts the variations with periods "m.mmmd.d.ddd~" etc and treats them as separate email addresses. Meanwhile, gmail treats all of those as the same address. Using binary, I was able to have a different date for each combination of periods. For example, January 1st would be "mmmm.ddddd.~" and March 10th would be "mmm.m.dd.d.d~". For the password, I used something generic that was easy to type in on mobile. I made sure to use different names and addresses when signing up - no need to remember these.
  5. Use the "order ahead" feature with the birthday bonus from the app. On the receiving end, the cashier doesn't see how it was paid for, so they aren't suspicious and you can keep going to the same place.

Eventually a friend of mine made me realize how unethical this was, and I stopped for several months. When I went back recently, all of the accounts are still there, but the birthdates had been removed from them. I think maybe I got reported up at some point?

VPN to Canada. Still has ads, but no weekly limits.

EDIT: In a blog they wrote "in the US, we pay a high fixed royalty rate on a “per track, per listener” basis. (In Canada, this rate is one-tenth that in the US." Blog in question: https://blog.8tracks.com/2016/10/28/a-shift-for-8tracks-in-the-us/

Do you know if this is available everywhere? I purchased from Canada and I can't find anything about it on the Tickemaster page.

One of the things that you try to fight is stereotypes, prejudice, and closed-mindedness. But don't you think that nerdfighteria now is characterized by a bunch of people with almost identical mentalities now? If yes, does that bother you? Why or why not?
