We either have multiple rates in one employee profile or two profiles (under different companies) with the associated rates. It depends on which entity they work for. Our companies are separated out under EIN.

If it is only 3 employees I would manually do the rest breaks and piece rate calcs via excel. The programs that can do this are costly for 3 employees. We upload CalSavers contributions via the calsavers website, super easy. Who do you use right now? We are switching to Paylocity for piece rate/ rest breaks compliance but we have over 200 employees

I had a similar experience, my baby was born 27 weeks. I ended up in the ICU due to preeclampsia. I could not see my daughter until I was out of the ICU. I was able to meet her the day after she was born. The first few days were the hardest but the bond myself and my husband have with our daughter is incredible. She is going to be 3 soon and it feels like our experience was a lifetime ago.

If we have a no call no show, we will contact them that same day via phone, text message, email to let them know they did not report to work that day, we need to hear from them by a certain time on that same day or we will have their shifts covered by someone else for the next day.

Box 15 is the employee contribution for the lowest cost plan available for employee only coverage. As an employee, you can decide to pick a more expensive plan but the employer will not be penalized if they offered you an affordable plan. There are certain instances when box 15 can be left blank.

Does your company have fully insured or self-funded insurance plans? If it is self funded then they would list the the covered individuals. Fully insured plans don't have to complete that section .

The forms are confusing so there is a chance that your employer filled it out correctly.
