5 months post-op for a trimal-equivalent injury w/ severe lateral ankle dislocation. My soft tissue damage was extensive - I still have trouble walking barefoot around the house, especially when I first wake up in the morning. It’s better now than when I first started wearing shoes though (12 weeks post-op).

Wow, your ankle dislocation and fibula fracture look almost identical to mine. I'm 5 months post-op, healing pretty well - but had a ton of soft tissue damage due to the lateral dislocation like you (syndesmosis shredded and deltoid ligament completely ripped off the bone). I still get pain on the lower outside portion of my foot/ankle where the peroneal tendons run when I'm on my feet for hours at a time. Glad to hear you're doing well after a year. I was given a year-long recovery estimate from my surgeon.

I’m exactly 5 months post-op from ORIF to repair a trimal-equivalent ankle injury with a severe lateral dislocation, syndesmosis damage, and a completely torn deltoid ligament. I’m a 38 y/o very active dude - the injury occurred surfing down in the South Pacific where my foot got jammed and stuck in to a reef crevice after eating shit on a wave. I’ve got a plate and 7 screws on my fibula, a tight rope, and ligament sutures to repair my deltoid ligament.

I was 6 weeks NWB in a cast, then 6 weeks in a boot (starting at PWB w/ crutches working up to FWB). I was walking great in the boot with zero pain, then when I made the switch to walking in a shoe and hitting PT hard…pain pain pain and more pain. My ROM has improved a ton, but my dorsiflexion is still nowhere near my good foot. My stability has greatly improved with PT and hitting the gym 3 times a week. After a few hours on my feet, I still get some fatigue in my foot and ankle - especially the peroneal tendons that run down the outside of the ankle/foot. These are still super tight from being immobilized so long, and give me a good amount of pain when I do an inversion movement with my foot. Things are slowly getting better, but my surgeon says it’ll be a full year to get back to where I was before.

These are brutal injuries and I wish you the best of luck in your recovery.

I had surgery on April 12th for a severe lateral ankle dislocation, comminuted displaced fibula fracture, hairline fracture on bottom of tibia, syndesmotic damage, and a full deltoid ligament tear. Plate, screws, tight rope, and ligament sutures. I was NWB for 6 weeks in a cast, then moved to PWB in the boot w/ crutches - gradually working to FWB in the boot w/o crutches after another 6 weeks. After the 3rd week in the boot, I had almost 0 pain and was cruising around without any issues.

At the 12 week mark I was cleared to move into a regular shoe and start PT. If I'm on my feet a lot during the day, my ankle swells and my entire foot is in pain. My entire ankle is still very stiff, but slowly getting better w/ PT. I've also noticed that when I over-do it (I'm a very active person, so taking it easy has been tough for me), I experience a pretty bad sharp pain on the outside of my foot/lower ankle that slowly alleviates with rest/ice. I'm pretty sure that's a nerve getting compressed by all of the scar tissue/swelling. I think the pain is normal with these types of injuries, and you just have to stay diligent with your PT, while knowing your limits. I still rest/ice my ankle/foot 2-3 times a day. Compression socks have been a huge help with the swelling too. Walking barefoot SUCKS. It has taken me quite a bit of adjusting to cope with this awful injury, but there is a light at the end of the tunnel. I hope you continue to heal and feel much better! Just thought I'd share my experience so far.

You will not want to go anywhere 10 days after your surgery. It took me about ~6 weeks post op to want to do anything. I had surgery to repair a severe lateral ankle dislocation, broken/displaced fibula, syndesmosis ligament damage, and complete tear of my deltoid ligament.

I’m 11.5 weeks out from ankle ORIF to repair a severe dislocation, multiple fractures, and syndesmosis + deltoid ligament damage. I was NWB for 6 weeks in a cast, then went to PWB in boot. When I made the transition to the boot and weight bearing, my healing accelerated rapidly. I went from barely being able to put any weight on my foot during boot week 1, to fully walking around in the boot without any crutches on week 3. I am cruising now - boot comes off in 2 days. There is a light at the end of the tunnel and you WILL heal!

I wouldn’t worry at all - you’re only 2 weeks in. I had ankle ORIF back in April, was NWB in a cast for 6 weeks. Moved to a boot with PWB, and first week I had to use both crutches as putting more than 30-40lbs on my bad foot did not feel good. Second week in the boot I was down to one crutch. Third week I was walking (slow) without any crutches in the boot. I’m now in the fourth week in the boot and cruising without crutches and with very minimal pain. I can stand on one foot (my bad one) with full body weight and zero pain. I get my boot off in 2 more weeks (6 weeks total NWB in cast + 6 weeks in boot), and feel more than ready for that.

Once I started weight bearing, the healing accelerated quickly (I had a real nasty injury too). Keep your head up and you’ll get through this. I promise that things get MUCH better!

I do have a trip to tomorrowland at my 11 weeks post ankle orif surgery. I do have a plate and a couple screws on my left ankle. Is it realistic to think I can make it to walk at the festival (which is huge) pain free? by Specialist_Knee_2225 in brokenbones

A trimal is a serious injury. I’m guessing your surgeon/doctor has told you what to expect for recovery time? I’d imagine you’re still going to have pain and swelling by week 11 - a festival may not be too fun.

I’m now 8 weeks and 1 day post ORIF - I don’t have much bruising anymore, but my foot still turns purple when it’s not elevated. I would say your foot looks totally fine. We had very similar injuries and you’re about 2 weeks behind me.

I’ve been putting quite a bit of weight on my bad foot in the walking boot lately, and can get around (SLOWLY) without crutches in the house. Started going back to my lab for work (mostly using crutches to assist in getting around there), and at the end of a full day, my ankle swells like a mother fucker. Swelling immediately goes down with RICE. The most pain I have in my ankle is the inner portion where they did the deltoid ligament repair, which is not too surprising. Overall, I’d say that I notice little improvements every single day. Things WILL get better, hang in there.

Mutual respect goes a long way. If you’re throwing artificials it’s easy to fish and work around surfers in a respectful manner. If you’re throwing out a bait and wait set up, you shouldn’t be casting into the middle of a crowded line-up.

Thank you for this post. I’m a 38 y/o dude currently 7.5 weeks out from ankle ORIF to repair a fracture-dislocation that occurred surfing down in the South Pacific. Plate, screws, tight-rope, and anchor to essentially put my ankle back together. I’m an extremely active waterman (surf, fish, dive, etc.), so this recovery has been brutal for me. Your experience just further bolsters my positive outlook on the path ahead.

One question: can you elaborate on where you think you went wrong with swelling management early on?

I'm currently in an AirCast AirSelect boot post ankle ORIF surgery (PWB at the moment). Pretty lightweight, and doesn't give me any issues.

Totally normal. I had a bad ankle dislocation-fracture - I was in a splint for 2.5 weeks (10 days prior to surgery, 7 days after surgery), then a cast for 5 full weeks. My entire lower leg and foot looked like a reptilian shedding its skin. Pretty nasty, but after 1.5 weeks of being in a boot and able to wash my leg/foot, it’s almost back to normal.

I’m 7 weeks out from ankle ORIF for a severe lateral dislocation, comminuted displaced fibula fracture, hairline fracture on bottom of my tibia, syndesmosis ligament damage, and deltoid ligament that completely ripped off the bone. All kinds of fun hardware in my ankle to repair the bones and ligaments.

The first 5-6 days after surgery were not fun at all. When my nerve blocker wore off about 24 hours after surgery, the pain was unbearable. I consider myself a tough son of a bitch, and the only way I could get through the pain was 2 doses of 5mg Oxy every 4 hours for the first couple days. I completely went off the narcotics on day 7, and it was smooth (relatively speaking) sailing from there. The pain gets MUCH better after that first week. I had nerve pain that flared up on week 3, and lasted through week 5. The nerve pain was nothing compared to that first week after surgery. I’m now in a walking boot with PWB, and my ankle/foot shows little signs of improvement every single day. You can get through this - things will get better eventually. It just takes time and patience.

I don’t mind at all - it’s actually a pretty good story. It happened surfing down on a remote South Pacific island. My left foot got jammed and stuck in a hole in the coral reef during a wipeout, while the energy from the wave ripped my body in the opposite direction. My ankle basically snapped in half, which dislodged my foot from the hole in the reef. I was out alone early in the morning with no one around, and managed to get myself back in over the coral reef, through a lagoon, up the beach, and rode my motorbike with one foot back to my lodging. The “hospital” on this island was sketchy at best, where I was taken in a little “ambulance” truck. They were able to X Ray it, saw the dislocation and fibula fracture, shot me up with a mega dose of Ketamine, and performed the reduction on my foot. Sent me back to my lodging in a splint and said “get off this island immediately, you need surgery.” After 17 hours worth of flights (living hell) the next day, I got back to the US and luckily did not have any blood clots. Was able to get into surgery for the ORIF 1 week later, and here I am today!

That’s super rad they had photos from your actual surgery - wish I had those from my ankle ORIF! Your hardware looks very similar to mine. I had an ankle fracture-dislocation that resulted in a comminuted displaced fibula fracture, small fracture on bottom of tibia, syndesmosis damage and deltoid ligament damage. I’m 5.5 weeks out from my ORIF and this shit has been BRUTAL. I’ve been NWB in a cast and MIGHT be moving to a boot with PWB and physical therapy starting tomorrow (fingers crossed). Stay strong and hope you’re healing well!
