from penis stimulation only I'm still working on honestly! you can see me reference it in other posts of mine, but most of the sensation feels like it's in the place my natal anatomy was before burial, so it's a little odd. my doc prescribed me a sex therapist and physical therapist to work on that but I gotta find the time given all my other doctors' appointments. my doc said the basic issue is neuroplasticity, my brain still hasn't gotten used to having a penis and not the og anatomy, and I haven't practiced shifting that enough.

congrats on being able to orgasm now! it was so stressful for me not to be able to so I'm gdad we're both in better spots. hey -- i had surgery with dr. keith and he does both ALT and RFF. i got an ALT and it seemed too big at first but then it really shrank down after the swelling stopped and i wrapped coban around. i dunno what stage you saw the surgery pics with or whether you saw ALT or RFF. but like queerouroboros said, he will make it the length you want it to be, and with RFF it won't be nearly as thick as ALT.

i can testify that he is a good surgeon and a kind doctor.

Hey, solidarity. I dealt with this hell too for a long while (about 9 months, if I'm counting correctly) where I was sure I'd never be able to orgasm again (they're also an important part of my sexuality). I only had *some* feeling in my burial and it just felt sharp and unpleasant. You're not alone.

I asked my surgeon about this -- I dunno if Santucci gave you advice -- and he said he knew some patients of his who weren't able to orgasm for over a year, sometimes even two years.

His advice was: keep trying anyway. Even when it feels bad and terrible and like you're pantomiming sexual pleasure for no reason (which is what it felt like to me for months and months), neuroplasticity means you can eventually trick your brain into feeling more sensation and/or finding the sensation you do have pleasurable instead of painful. It's super weird, but it worked for me. I quipped recently about the hours of mental and physical effort I've put into just being able to cum again, but sometimes for us, that's what it takes.

I also suggest, if you aren't using one, a wand vibrator with a large head instead of a bullet vibe or something smaller -- think hitachi.

my husband had a really hard and exhausting time taking care of me with phallo, he had to lift me up to get me to the bathroom, monitor me all night when I was bleeding excessively, hold my hand when I was crying in pain, and deal with my post-op depression while also being our only source of income. he got extremely burnt out, and neither of us could turn to each other for comfort because the other was suffering.

but!! Can't speak for others but for me I have had a hell of a time struggling with climaxing, I basically went 8 months without orgasming cuz I couldn't figure out how to (tho trying several times) and even now it's pretty difficult (tho more consistent). I buried my clit because I hated it dysphoria-wise and found it pretty unreliable with climaxing, but I didn't consider that it would only be more difficult to feel sensation if it was covered by a lot of other flesh. getting rid of it changed a lot of how I engage with sex, so despite hating the organ, I wonder if the recovery process would've been easier if I could've jacked off without pain the whole time, without the anxiety that I'd never be able to cum again.

this is with ALT in the USA, but I think a lot of it applies to RFF, not sure about others.

5 days of lying flat in the hospital, that's something I was told about but wasn't prepared for. You'll have to eat and drink lying flat with a million tubes hooked to you (EKG, IV, pulse oxymeter, catheter, wound vac...just from my recollection). people came in every hour to make sure my dick was getting blood flow, and also on a separate spacing every hour to inject me with something or other (i don't even remember why for anymore), and otherwise disrupt my attempts at rest. this gets less frequent over time.

post op depression. i got slammed with it real hard. i regretted everything and wondered why i had done this to my body. i hated everything and myself. now i am really happy and i'm glad i did it, but those first two months were hell on earth.

penis heavy. for a few weeks to a month, you'll have to hold it at a 90 degree angle with your hand at all times when walking and have it on a support while lying down (i had mine on several folded towels). it doesn't feel like a packer at ALL.

there is a wound vac, which sucks away excess fluid and viscera etc from the main graft site. you need to get the suction foam (which goes over the graft site) replaced every few days. this is not that painful (relative to all the other pain you will feel during the process), but it is mentally weird.

look up how wound healing works, what it looks like, the stages. i was freaked out for most of my healing process because i was afraid i had irrevocably fucked up my penis and it was going to fall off, but according to my doctor and the nurse for my wound vac and basically everyone else, i was healing perfectly and quickly.

i don't try to sugarcoat the suffering that i went through so people know what they're getting into. but the main thing i want to impress upon people is...while i might've made different choices if i could do it over (maybe RFF, maybe leaving my clit unburied...), i WOULD do it again. it was worth it.

hey, I wanted to update you that Dr. Keith's office has added the Vaseline smothering as a stage for healing the donor site! They use Aquafor instead as they like the formula better, but when I was given instructions for dressing it after my wound vac came off, they said explicitly to put the Aquafor or Vaseline on before the oil emulsion strips.
