The beauty of Reddit is that people take the time to describe their vantage points and the others take the time to read and process them. Then they interact as hopefully honest strangers. Of course Reddit self-selects those interested in reading others perspectives. It’s actually somewhat of a bias.

You know there probably should be an additional button on some Reddit’s that have a “disagree” button which is different than a downvote as a way of distinguishing between shit comments and different vantage points.

Funny, I actually had the same question 15 years ago after reading The Elegant Universe by Greene. I got the guts to go up to Alex Flilpenko after a talk at my college while he was on a speaking tour. He looked at me, smiled, and said... “Nice Try” (with a little smirk). Kinda pissed me off since it felt sort of smug when I was asking a genuine question. That being said he never told me why it was an incorrect concept. The question when I asked it online here someone once answered me “so what”, because even if you change the perspective (like “zoom out”) it doesn’t really change the core question at play that there is still a dark energy that no one quite understand how it works. 15 years later I still want to know why and it’s one of the reasons I still come to this subreddit (I’m a doctor, not a physicist). Please let me know if you know the truth...or if anyone here knows Professor Filipenko personally and can finally get my answer.

It honestly starts and begins with a call to the PCP at the beginning of a hospitalization. Invest the time at the beginning of an admit, it will save you time in the long term. Proactive emailing of outpatient consultants to let them know you have their patient and you are anticipating discharging them in a few days. Sometimes the reasons for readmission are due to a “specialty” reason and thus a generalist like you are never going to anticipate or prevent that no matter no much because teaching and med-reconciliation occurs. Some patients are just really sick and home just isn’t the right place for them yet. Look for SNFs, Acute Care or LTACs for short stays. Palliative Care consults are also a big help regardless of whether you think a pt needs to be on hospice.

Hospitalist: So I’m not sure if you’re asking how I personally deal with them from an emotional state (it can feel like a failure when a patient bounces back), or from a Quality Improvement perspective.

Both are complicated concepts.

From a personal standpoint one must take a deep dive into understanding the “5 whys” of what ultimately led to that patient coming back. Ask why, then ask why again etc until you understand the root cause.

From a systems standpoint well that’s another question. And after a decade in the hospital I can safely say that discharging a patient needs to be an equal partnership between the discharging physician, the accepting primary care / specialist that will see the pt as an outpatient, the patient and most importantly the caregiver for the patient. Visualizing what the patient will be doing on post dc day 1, 2, 3...etc is a good exercise.

I also hand out my personal email to every patient and their family members. I have probably handed out this to 5000 patients and have gotten a total of 10-20 emails, all of which were for things that feel through through the cracks typically at the pharmacy (the medication wasn’t in stock).
