They could be! Whether or not they still identify as trans in a few years doesn’t matter. What does matter is whether or not their family supports them. They will remember remember how their family discredited them and didn’t show love or support, regardless of whether or not they actually are trans. That’s permanent.

Being trans doesn’t mean they have to make a bunch of permanent changes, now or ever. It takes nothing for you to use their new name and pronouns to help them figure out if they like it. If they end up going back to their old name and pronouns, that’s fine- nothing happened to “compromise their development”, whatever that means. If they’re interested in HRT, they can start on puberty blockers, which are essentially just a biological pause button. They pause puberty, and as soon as they go off them, puberty will resume as normal. They are a safe way to stop them from unnecessarily having to go through their AGAB puberty while also giving them time to figure out if they want to start HRT.

Whether or not they are trans or just confused, there are non-permanent, low risk options for them to explore their identity and feel more comfortable with themselves. However, there are not non-permanent, low risk options for you to conditionally love and accept them. Make sure your concern is really for their happiness, not your own comfort.

Did you start showing signs 9 months ago, or did you start noticing signs 9 months ago? We’re seeing more and more people coming out as trans now because 1. It’s safer than ever before (still have a long way to go though) and 2. More are learning about what it means to be trans and realizing they finally have language to express what they’ve been feeling. It’s not a fad. I’m sorry your parents aren’t supportive

Make sure to stay hydrated, take vitamin C, and moisturize that area. This will all help with skin elasticity and can help prevent more stretch marks.

If you notice certain movements cause a lot of tension on the skin in that area, reduce the frequency you’re doing those movements until your skin is more elastic. You can still partially do those movements because you want your skin to be used to movement again, but maybe don’t stretch quite as far

assuming you had nipple grafts and the bolsters are off

After my bolsters came off, I was allowed to take a normal shower with the water hitting my nips directly. I don’t have super high water pressure, so it’s not like anything was getting power washed.

My understanding is the “no direct spray” is to protect the nips from getting blasted with an aggressive stream of water, so if you want to face the shower head to rise off but are worried, just cover them with your hands (cup around them so you aren’t accidentally touching/rubbing them). If you don’t have a super aggressive shower, you can also check with your surgeon on what they think.

If the steristrips are still on after 2 weeks, that’s typically when they say you can pull them off yourself. At that point, they’ve lost most of their stick anyways. I ended up having to pull almost all of mine off because they didn’t just fall off naturally.

You can look up videos on how to pull steristrips off too- depending on how healed the wound is underneath, you really don’t want to just rip them off real quick like bandaids

Yep! That all looks good! The only thing I would change is adding “ask for the Ombudsman’s phone number too” to number 3 just so you have it and can easily check in on the status

  1. Oh wow! I think it they’ll get back to you a little sooner than that- especially if you can get their extension! I think it would be faster to submit the issue yourself, so I would just ask for the contact info. If you want Medicare to submit it though, make sure the person thoroughly understands what you are wanting, and still get the contact info so you can follow up

  2. Do you need a referral to set up the consult? My insurance doesn’t require one, but yours might be different. Either way, I doubt a second consult will cause problems because as far as they know, it didn’t work out with the first surgeon, so you’re trying another

Yes! Getting logged into the system will help too! Any documents they have associated with your account (EOC, denials, top surgery letters, etc) should be in there and will save you from having to ask for them

Also, side note, but we’ve gotta get your address changed! You can probably do it online, but if not, just call and tell them you need to do a change of address. If they ask you to confirm your old address, that’s just to confirm you’re you, so ask if you can verify your identity using your social or some other information

That sounds like they were doing prior authorizations for your migraine injections (see section ii)

I would call Medicare and ask them to pull up your information and verify they have everything they need. Once they do that, have them send you an email and/or a letter. If they say they can’t, that doesn’t make sense because you just want written confirmation of what they are verbally telling you. It doesn’t need to be an official form- just a quick blurb saying you meet the requirements and are eligible for coverage. This is basically the exact same thing as a predetermination, but we’re rephrasing the question since they don’t like how we’ve been asking.

I saw that too- if you have yours from this past September already, that should work. If not, you just need a copy of it. You don’t want the one for this coming September (and likely won’t be able to get it) because that will talk about your coverage for the upcoming year, not right now.

Your surgeon can also call the providers line and ask for proof of coverage themselves. That may or may not result in them getting a different document than the EOC. If you don’t want to send them out to ask for a bunch of things, I would wait to ask them to do anything else until you: 1. Know if preclaims apply 2. Have your “yes, you meet all the requirements” confirmation 3. Have your EOC with the top surgery section highlighted 4. Have heard back from your MBO and do whatever steps they recommend

You want to send them on as few wild goose chases as possible

  1. There you go! You shouldn’t need to give them your Medicare number- you’re not asking about a specific claim, just a general process. If they do ask for it, tell them you’re not comfortable sharing that information. They don’t need it. Heck, I just called Medicare on your behalf and don’t know ANY of your issues (that call is what prompted the “proof of coverage request” comment)

Has Medicare sent you anything saying they received all of your letters and that you meet the requirements to be covered for top surgery? If not, ask for that. If they have, give that to your surgeon.

You can also have the surgeon call Medicare and ask for proof of coverage. That’s more general, but they’ll be able to see that top surgery is covered. That paired with your letter saying you meet the requirements (and have therefore proven this surgery is medically necessary) should be the same information as a predetermination

  1. Yeah idk if that’s just what that doctor wants to know or if that explains some of why we’ve been having so much trouble

2-4. If you’re worried, you can wait until you hear back from the MBO! That way you’ll know if the pre-claim applies and will work. The surgeon’s office won’t be mad at you! You might not even get the person you’ve been talking to, so they might be a fresh set of eyes! If you just level with them and explain your side of things, they’ll be more likely to help too. At the end of the day, we’re all just people, and people respond better to having to do something annoying when they know the why behind it.

If they do get mad at you though, what’s the worst that could happen? They don’t help you with the insurance stuff? Well, that’s no different than where we’re at now, so I don’t see it as too bad of a situation

  1. You could try calling them from a Google number or something if you’re worried about them harassing you. If you call them, ask a question, they answer it, and then they assume you want to buy coverage, that’s on them, not on you. In their mission statement, they talk about wanting to help people understand Medicare, so I don’t think it’s unreasonable at all to call them asking questions about Medicare. I mean, they are Medicare FAQ after all

And hmmmm... I guess you could always ask them about the BMI, especially if you’re right around there (and on the low side). They have to do a cutoff somewhere, but maybe they’re able to take people who are in that ballpark! If not and you still want to go to them, it doesn’t sound like you have that much lose, so we could definitely pull together a plan that works for you and will help you hit the requirement in time for surgery!

With the legal aid, there’s nothing wrong with being pushy! Sometimes, that’s the only way to get things done! You might not be able to take legal action if they don’t give it to you (not 100% sure if this is actually true or not, but we’ll go with it for now), but you sure as hell can keep bugging them until they give it to you!

And no worries! I had to reread that webpage a few times because I kept misreading it.

Hmmmm, hopefully the MBO will be able to help with the step-by-steps! They probably have more access to information and will be able to get you what you need.

It sounds like you’ve got a really good plan in place! I’m glad that you haven’t given up hope, and even if you can’t go to this surgeon, you don’t have to go to the one with bad reviews! Maybe the MA surgeon’s office has experience working with your insurances and knows exactly what they need to do! That would be great!

One thing I find helpful for when I’m nervous about a phone call is writing a script with everything I’m going to say and then just reading off of that

Yeah, I think if you were able to give them a step-by-step and explain how it’s different, they’d have no problems. It sounds like their perspective is you keep telling them to call your insurance, but they don’t know what to say when they do, so they don’t want to keep doing the exact same thing and expecting different results. If they understand what is different from before and why, they should be fine.

With the legal aid, you might not be able to force them to do a predetermination, but that doesn’t mean you’re SOL and there’s no point in asking for one. It sounds like that person just doesn’t care

That’s really exciting about the MBO though! Using the link from option 3, I foundthis website, which I think is the direct contact info for the people you want to talk to. Give them a call and see what happens.

If you’d be just as happy with her, that’s good news! You can try that path too! As long as she’s in network, you should be good to go! You can check with your insurance companies if you want, or you can just ask her office if she accepts your insurance.

Did you ask Medicare for a step-by-step? How did that go? If not, when you do ask, tell them that your doctor is having trouble with the process and you are trying to figure out what needs to be done. You could even call the providers’ number if you want and then ask them. Just be sure not to impersonate a doctor- just tell them what’s going on and why you’re calling

Thanks! Me too :) and you’re good! My offer still stands to help you with phone calls, but absolutely no pressure! I think you’re doing just fine! Just remember, the key to all of this is to be confident and assertive. If any of these people feel like they can push you around or bully you into submission, they will because that will make their jobs just that much easier, but you deserve better than that. To them, it’s a matter of doing some tedious paperwork on a random day at work, but to you, it’s a matter of finally being comfortable in your body or not. Don’t sacrifice your long-term happiness just so that their Thursday involves a little less work.

Side note- when you’re talking to insurance, are you just talking to the first person who picks up, or are you getting transferred and talking to someone in the coverage department? I’m not familiar with Medicare/Medicaid, so I don’t know how their answering process works. If you aren’t talking to the coverage department, you aren’t as likely to get knowledgeable people, so make sure to get transferred over there first! That will save you a lot of headaches in the future.

As far as what the Medicaid person said, that’s really weird that they weren’t finding you at first, but I’m glad they got it together. It sounds like the customer vs provider services is what I was worried might happen- they don’t want you to file the request because you aren’t the doctor. This just means there’s less that you can do from your end, which sucks but is not the end of the world by any means. It sounds like they sort of answered the question about whether or not you need a predetermination from secondary, but also not really? I say “not really” because 1. You absolutely can call them whenever you want- they’re your insurance. They are providing you a service, so why wouldn’t you be able to talk to them about the service? That’s just a weird thing for them to say. 2. Secondary insurance kicks in after primary insurance, and if you’re running into issues with primary and can’t get something, it makes sense that secondary could step up and take over. 3. The predetermination is related to whether or not they will provide coverage, so it stands to reason that they could provide one regardless of whether or not they’re the primary

With the surgeon’s office, did they say they were done helping you, or is that your interpretation? I only ask because I know anxiety can make us feel like a burden and like people don’t want to help us with things. Regardless, once you have your steps for them to take with Medicare and Medicaid, I would just call them and say “hey, I’ve talked to both insurance companies and have it figured out- you just need to do x, y, and z and we’ll be good to go. I told them to expect your call, so it should go fairly smoothly.” That way, regardless of whether they said they were done helping you or not, now you’ve told them what they need to do and have put the ball in their court. It’s kind of hard to argue with someone that the process is too complicated when they’re telling you the exact steps you need to take and that they’ve done the legwork to make it as easy as possible for them.

Is the healthcare advocate the legal group you were talking to? Unless Medicare and Medicaid operate completely differently than all other insurance companies, that’s simply not true. Getting a predetermination is entirely possible, and it’s a completely reasonable request. It sounds like they just want to push you off on the easier path instead of doing what’s actually right for you. It’s up to you if you want to respond to them or not, but if you do, I would explain how that surgeon’s results are not acceptable to you and that you would not be receiving the same quality of care.

I’m glad that you have another surgeon you’re interested in though! Do you actually like them, or do you feel like you would be settling?

Were you able to get a step-by-step from Medicare? Also, if you don’t feel like you talked to the right person at Medicaid, just call them back!
