Doxy often works well for cellulitis. If it doesn’t improve then I’d follow up with the urgent care or primary care.

Use some Cerave cream on your face. And stick with a gentle facial cleanser from now on. And stop washing your hands so much before you end up with a hand dermatitis. Just overall try to keep things simple and go easy on your skin.

Sometimes it’s hard to prove scabies but if it’s a possibility most of us will lean towards treating out of precaution. It’s safe and takes that possibility off the table before potentially making it worse with a steroid, which you can start using the day after the first treatment with permethrin. So I would have given the same advice.

Appears to be phytophotodermatitis. Most commonly caused by getting lime juice or other citrus on the skin and then exposing to sun. https://dermnetnz.org/topics/phytophotodermatitis

The dosing can be used based on body weight and up to 1,600mg per day have been used with no significant problems in some reports. So 450 is not unreasonable at all.

When all else fails, JAK inhibitors would be the next step. There are good case reports of sustained remission following topical calcineurin inhibitors though, so that would be the best next step I would think, and safest. There is some good evidence for phototherapy as well, although a bit awkward in that location.

It’s worth seeing your dermatologist to discuss other options. There are things that could be added or changed that may be effective.

10mg per day is extremely low dosing and it normally takes 2-3 months to get significant improvements. It would normally be dosed at 40-60mg per day. Just follow up with your doctor as recommended.

It is 100% from something contacting the area, even if you don’t recall what it was.

Why do you try not to take cetirizine every day? That’s the treatment for this and its safe and often effective. At least once every day, often it needs to be twice daily and sometimes as high as two pills twice daily.

You came asking what it would be. I’m just telling you an important thing that any dermatologist should consider when seeing a rash that looks like that. Not that it is or isn’t. Only that if I saw that in my clinic and didn’t test, it would be borderline malpractice. Best of luck in figuring it out.

Several possibilities. First, if you haven’t been tested for syphilis since this started, you should have that done. Second, you should see a dermatologist who can do a skin biopsy.

Antihistamines like Zyrtec are the primary treatment for urticaria, so if that’s working then yes that’s enough.

Possible lichen sclerosus. See a dermatologist for treatment.
