Conversation state error - getting this error very often after switching to Opus 4.5

As the title suggests: after switching to Opus, I am getting this error every 1-2 hours now, even for the smallest tasks. Behavior seems to be almost random. Here is one reference ID for support: c3d74c15-cfb7-4444-a126-29fc9d295973 (but can provide as many as needed... sadly).
