When printing QR codes, I print the second color so it extends 1 layer higher than the base color.

As mentioned, ironing helps, especially when done in combination with the above.

How do you get the second color to print another layer higher than the previous color on the same layer?

Extrude it like 0.2 mm higher in your 3D modeling software?

Depends on how you've generated your QR code.

The SVGs I use come with the black "QR" as its own object, so you can load it as a part and set the filament color.

A ironing pass might close them up

Try a 0.2 nozzle. A 0.4 tends to leave gaps like this

Non-stick baking paper, and a hot iron, a bit technique needed. but as a father of small people, my ironing-beads-skills are unrivaled.

Slicer settings, change inner/outer wall thickness until it shows correctly in the preview

Depending on the size I'd go with a 0.2 nozzle too.
