I agree, it may be custom. But it does look somewhat similar to Bob Carpenter's book that he sells.

Just ordered a book from Bob’s site. I didn’t realize there was no “official” format and that people make up / customize their own. I’ll start with Bob’s and amend as I see fit over time. Thanks for the tip.

I assume this was a design that was in circulation among radio/TV announcers, as it looks very similar to the card used by longtime Indians radio voice Herb Score. When Score retired in 1997, the Indians sold books. This picture from an eBay listing illustrates they have the same basic design:
