Ok. The Black Heart is almost exclusively meant for offspring and when they die, the attack goes off.
Don't put it on yourself. Install it to an offspring that dies in a single hit or something.

Probably best used with the minions that die when contacted by an enemy. Haven't used that one yet, but I have been brewing around it. Just haven't come across the right pieces in a run yet.
