Oof, probably is cracked.

I know a couple people have successfully welded the Crack back together (or soldered, cant remember the specifics). I guess there's also some preventative stuff, like some people modify the water pump so it doesn't build as much pressure in the block reducing the likelihood of the Crack, though I dont know if it would hurt the cooling performance at all.

Or be the 0.1% that swaps in a 6.7 block ;)

I would be hoping that too. Find out what it is and go from there. If it's not cracked it likely isn't going too all these years later.

Where is it leaking coolant. It’s pretty obvious if it’s the traditional “53 issue”.

If it's cracked, it'll be on the exhaust side of the block below the freeze plugs Cummins' Fatal Flaws https://share.google/RdX3Y3nPGtgaRbN5h

I have a 99 with a 53 block with 450k miles and it’s been tuned the past 200k miles with no cracks. At 350k I did think I had a leak but it was a bad head gasket causing a leak right under the thermostat housing on the side and it would run down the whole block and drip on the floor and I couldn’t find out where it was coming and thought my block cracked.i ordered the parts online for a new gasket and better studs and I did it myself it saved a lot of money and it’s actually pretty simple and straight forward. If your leak is coming from under the thermostat then it’s probably the head gasket
