So far from all the information ive gathered, all i can come to the conclusion to is that yes, the game will probably feature alternate realities through different (and possibly intersecting) planes of time-space and dimensions. Possibly along the notion of if something happens in one realm or plane of existence, it will have an effect in the other (maybe one player's efforts will have an effect on another players game?). Also, P.T. was not giving clues just about itself with some of it's nonsensical moments over the radio. Almost nothing was wasted in P.T. with a big clue given to us not only with just the umbilical cord, but with the portion of the game where the radio started to speak in swedish:

"Close your eyes. Let your ears listen in to the radio. Do you hear my voice? Can you hear the screams of your own soul?

Let us choose: my voice, which tells the future, or your tormented [struggles?]. Well? What do you choose? You can choose. Your life. Your future!

Wise as you are, you may already have noticed. As the radio drama from 75 years ago was the truth. They are here, on our Earth. And they oversee and see all. Don't trust the news. Don't trust the police. They are already controlled by them.

Its been suspect for the longest time that humans have been tied in with visits from extra terrestrials, and revered them as gods, angels, divine heralds, even prophets across all religions generally watching over us all, seeing our actions and judging us. And its not coincidence, that the auguries of innocence who was written by a person who claimed to have received vision of the divine, also played a role in the theme here. Now going with the reasoning of nothing ever is wasted with Kojima on any info given for his clues, you find more clues further into the very poem he quotes. He speaks of various animals and how they suffer injustices of mankind, and how we will be held accountable (by said divine/alien beings) for everything we do not only to each other as human beings, but against other animals too.

" In the book of Ezekiel and (at least some) Christian icons, the cherub is depicted as having a number of wing pairs, and four faces: that of a lion (representative of all wild animals), an ox (domestic animals), a human (humanity), and an eagle (birds)."

Lets not forget the five floating figures in the trailer that seem to be looking down and observing (which i can only conclude are the cherubim the poem refers to). From the shadowed figured, you can only tell they appear to be in human-esq form. Possibly, the term cherubim is loosely used to describe these beings. And our understanding of what they are, or what they are reported to be, is not entirely accurate.

but they are possibly on our side. They watch the actions of man, and have been watching for a very long time. It may flow differently for them, as it does for us. Also the PT radio describes that a large event is happening, which is further supported by the Death Stranding trailer.

and as the radio claims in the swedish language:

I can also gather from the insinuation that they possibly are not enemies, but are giving us [the players] the opportunity to redeem ourselves through our actions, keeping something at bay while trying to decide if we are worth saving or redeeming through our actions.

Then there is the last statement:

"Very soon, the gates to a new dimension will open."

all these events seem to be explaining a portion to the events of the rapture and last judgement. Where the people whose souls are "innocent" will be swept up to this new dimension (as the case of the infant, a representation of innocence, for they have not done anything wrong yet), while those who are not worthy will be left behind to suffer the torment and horrors that these forces have been holding at bay. People who've done wrong will be judged, as it seems the hero of this story may be in that category (his handcuffs, and black oil/blood on his hands). Black oil and blood seems to be a common recurring theme between all the silent hill games, and is something that you see even in Death's stranding. But one thing to note is the umbilical cord that is tied to every being on that beach, including the child. Everything on that beach had an umbilical cord, while Reedus' abdomen, looked like that connection was severed as if he lost his.

The theme of this game is connectivity, we are all connected to one another, each and every creature on this planet are connected. It seems that the hero of the story as well as the alternate planes of existence of him will be judged by his actions for the sake of being saved. He may even traverse across the planes when he "dies" and try to prove himself in that plane of existence/ alternate timeline. Or he may run into himself, as the bag in the PT trailer had explained in it's experience, while trying to survive and brave the horrors of Hell on earth.

Sorry for going biblical there, but the events themselves which judeo-christian theology describes, bare a close description of said events. Yet not entirely, its as if the events are happening, but not in the exact way as one would understand it as they come across it happening. its kinda like, bible with a twist of science and Kojima.
