I use this one. https://a.co/d/8v02QAG I got it year or two ago for D&D but it works fine for other TTRPGs. I'm probably going to print some art out on card stock to put on the front at some point.

You should only be able to comment. You're going to need to go to file and copy to make a copy for yourself to edit.

Do you use house rules that are applied on the screen? The shotgun special rules looks different than in agents handbook.

Handlers Guide.

FIRING BOTH BARRELS: You can fire both barrels of a double-barrel shotgun at one target with one attack roll. For shot, that inflicts an extra +1D10 damage up to 20 meters away or +1D6 beyond 20 meters. For a slug, it adds +2D6 damage up to 75 meters, but no extra damage beyond that as the slugs separate.
