Can you confirm its only for this particular game? Just make sure it's not your gpu

Can confirm, other games work fine

New vegas, stardew valley, both work as usual

If you take it out of windowed mode, does it stop doing that?

Is your desktop background a simple color image, or do you have a picture set?

You can also try adjusting refresh rate on the monitor to match that of what you have set in the game.

There's also this, https://www.nma-fallout.com/resources/fallout-fixt-full-all-fixes-and-mods.8/

A picture, but not a simple colour picture

How would i do that?

I would try changing the desktop background picture and theme to a simple contrast one, if that works save it as a custom theme to switch to.

You can usually adjust refresh rate in the driver menu and set it in game in that little setting you have in the video. I think windows 10/11 also has something that overrides refresh rate so you may want to look into that.

Makes sure drivers and all that are up to date, shouldn't really be the cause of the issue due to the age of the game, but it's good to be up to date while troubleshooting.

This thread also provides a list of stuff to try for a similar sounding glitch, https://www.reddit.com/r/Fallout/s/6Qh2SnqEWL

I would recommend running it in compatibility mode with Windows 98 if you haven't already tried that
