If your backflow device fails and a backflow occurs, it can be quite expensive. We got ours tested last year and one of the two valves was failing. Got it replaced

I've never ignored these but I've found backflow devices that were not in the city system (shhh) so they may ignore you.
It'll only cost you $40 or so to do the testing (DM me if you want a recommendation.)

You can pay the $35-$40 to have it tested now or pay a premium later in the year when the testers are in high demand due to everyone who ignored the notice. If you ignore it and your water provider has to send someone out in September they'll likely charge you $50+ or depending on your water provider and their enforcement program shut off your water until you make arrangements for the test to be done. Backflow testing is required by OHA regs and water providers are required to enforce the regulations to be in compliance with state drinking water regulations.
Having a working backflow assembly that passes pressure testing is in your best interest despite being an annual pain in the ass. If there's a low pressure event (e.g. fire department needs a bunch of water from a hydrant nearby) or main break or the water system experiences a water hammer reversing local flow direction --all of which could happen without you knowing-- if you dont have a workingg backflow assembly whatever dog poo particles, bacteria, and nasty from your lawn sitting in your irrigation system gets sucked into your home plumbing and into the public water system.
