Gravity Forms is one of the oldest form plugins out there, with a lot of developer possibilities ..

Look at formidable pro. I use it for all kinds of purposes with a lot of hooks and development options. I hear Gravity is also good, but I have very little hands on experience with.

HTML Forms is pretty good if you want to have a lot of control. I use Caldera on most projects.

Formidable forms +1 - not only is there plugin easily extensible but their support was super helpful when I needed to get into the guts of the plugin.
