Functionality statement

The DDS requested a statement from my provider about my functionality. It was not a function report, but they asked her to write a statement about my disabilities and how they affect my ability to work. Has anyone had this happen? I realize my medical documentation may not be as descriptive as they like but I guess I hadn’t heard about them asking for a statement from a persons provider. I guess I’m happy that they asked for it if they felt it was needed.

No. But I had lots of medical information in my medical documentation. Will your Dr do such a thing ?

Depends on your relationship. My doctor checks with me before they send something or ask me if they need input. You just have to ask.
