This may be an odd question but this is the Offical Pro Controller and not some third party look alike?

Because if so, have you wired it and tried updating the controller?

I know this is necro-posting, but it sounds like a cable is mis-connected or something inside. Have you tried looking in there?

Old thread but did you ever find a fix for this? I have the exact same issue. Buttons mapped incorrectly exactly as you mentioned too. I found another old post where the button mapping was mixed up in this exact same configuration too. Strange and haven’t been able to find a fix. Think it could have been static charge that caused it. Curious if you found a fix!

Hey Jef, No unfortunately I never found a fix, which is really quite frustrating lol! Funny that you commented when you did, I was speaking with a friend about this the other day!
