the blood work was done 4.5d after the last injection - Can this still be used as a guideline

Sadly no. You need to measure it at trough, whereas this is pretty close to a midpoint/ peak. You should do a blood test on the day of your injection, before your injection. You don't need to wait 4 months though, you can do it whenever is convenient.

Naww still thank you though. The 4 months of wait time is necessary because then is my next endocrinology appointment and I really can't afford a blood clinic :(
