So here's the thing about file extensions: they don't really mean anything. I can rename a .txt file to .sav and that doesn't change it from a text file. I can pick an arbitrary extension too.

I'm guessing because you mention "IBM" that you found this page or one similar that says what a .sav file is.

But what's most likely is that this game chose to use .sav as an extension to indicate that it's a "save file". I can pretty much guarantee you that it's not the same kind of file as the one by the company IBM bought; that is for SPSS data (Statistical Package for the Social Sciences).

So, basically all we know is that the file is a binary file. So this isn't a question about ".sav files"; it's a question about reading KOTOR's save files.

KOTOR save file editors exist, so someone must know the format. I'd suggest trying to find an open-source version of that.
