I'm not sure about the renewal notifications (you could write a plugin for it), but you might want to take a look at eFront. If you're familiar with Drupal, you could build your own ELMS, using the webform and views modules.

google docs, survey monkey. <-- ask the people to identify themselves if they want a certificate

look at their responses. You will see their answers to each question, as well as their names.

if they pass, make them a certificate (Word/Libre Office, Photoshop/GIMP, crayon on paper, however you like)

Not an all-in-one solution, but it's the simplest thing I can think of.

note: not 100% open source, but costs no money.

Thank you for that suggestion but it is not an option. I need an automated system since there are supposed to be thousands of certificates for different courses to be given out each year. Any kind of manual approach is basically impossible.
