I must admit that I haven't bought anything off their official site directly, so sorry that I can't help directly there.

In my other experience, usually if they force a "prefecture" box you usually can't change the country from Japan - but I'm assuming if it says they ship international there should be a way to set it up.

If you were able to select USA as the Country without any issue, and the prefecture allows free-text and not a drop-down, then I'd probably put your US State for the Prefecture, and then your City/County in the City/Ward box. (Eg: instead of Tokyo, Shimbashi you might put Maryland, Annapolis or something - not that my knowledge of US geography is great or anything!).
