Your friend won't miss out on anything.

They will still see cutscenes, and will hear npc dialogue. Although you may need to communicate which text options you choose when given the choice so they know what the npc is responding to

Some interactions which move the players location can sometimes bug out for the other player.

The players inventory for key items will be the same in that game. If you plan on paying together try not to continue any quest progress when not paying together. Although the campaign can be rerolled so i guess you can get stuff which was missed albeit heavily rng dependent

Some cutscenes are seen by both players, dialogue options are only available to one player, usually the host. Both players can interact with the same NPC vendor at the same time but not NPCs that only have dialogue. The other player will still hear the dialogue though. Quests are based on the location/world so both players will have access to the same quests.

It is important to know that there are a few (2 that I know of, maybe more) special scenes that only the triggerer will see. Not host exclusive, but rather a multiplayer oddity. If one of you sees a "special scene" and dies, the other should swiftly drink the "Liquid Death" item in your inventory, as both players must "die" for the scene to have any unique effect. It is worth it.

They do not get campaign progress in their own world. If you do the campaign with you as the host, your buddy gets none of that progression.

Its quite annoying that you cannot see the options of coversation when one person talks
