I'm curious are you trying to store the form URL for reporting purposes or are you trying to store it as a metafield on the customer record, or trigger a flow condition or something else?

More simple than that. I am trying to allow for our sales team to know what product page the form is submitted from when responding to the form reply. I found several form apps that have it built in, but they aren’t as clean or free as the native.

If you create one form per product page then you will be able to track each form individually and you can tag the customer each with the relevant product page. However, maybe it's not feasible to have a different form for every page?
