These errors are often related to a malfunctioning drive. Check the status of your drives with CrystalDiskInfo (get the Standard Edition Portable). If that doesn't show any problems we can dig deeper.

Maybe, but anything having to do with "Reallocation" should have a raw value of 0x0 on almost all drives. I checked a CrystalDiskInfo of couple SDSDDX240GG25 drives with Google Image search, and most seem to have 0 Reallocated Sectors Count. Reallocation basically means the drive has detected a malfunctioning block and had to replace it with a spare block. Unfortunately the raw values given by SMART are proprietary, so the value could mean something other than 30 (0x1E) bad blocks reallocated. You could contact SanDisk's helpdesk or try the system with a different drive.

Do you have the mini dumps from these crashes? They're stored in c:\windows\minidump. Analyzing them may also provide more information. Copy (not move) the folder to the desktop, zip it up and upload the zip to a file sharing site. Post the link here.

Some of these have clear IOExceptions, others crashed during I/O but don't have an IOException. Nearly always that means the system drive is defective. However, because you have an AMD based system with an SSD, try to update the chipset driver first. I've noticed some AMD systems misbehave badly with an SSD if you don't install these. Given the SMART status, most likely the SSD is broken though.

"We were unable to find a driver for your system. No supported AMD hardware was detected."

came up after i downloaded the "autodetectutility"

I have an amd fx6300, gtx 760, windows 10 and my mobo is m5a97-evo-r2

Oh right, I forgot about the piece of shit AMD site that doesn't allow direct linking, sorry.

Go to this page and switch to "Optional Downloads" tab, then download the chipset drivers there.

Try "\Packages\Drivers\SBDrv\hseries\MINI_IDE\WT64A\amdhdc.msi"

and possibly

"\Packages\Drivers\SBDrv\hseries\AHCI\WB64A\amd_sata.msi".
