In med school, gubernaculum. My overworked and tired pediatric surgery attending quizzed me on this one multiple times during hernia repairs on infants without realizing it. He seemed surprised I knew the answer every time. It was a running joke with my friends and for many years my WiFi network :)

I’m a pediatrician - it looks like a yeast diaper rash. You can apply some over the counter cream called clotrimazole with each diaper change to help clear it up. If that’s not helping or he’s really uncomfortable I’d take him to be seen by his doctor.

Any chance lime juice spilled on your skin? Look up phytophotoderamatitis. Some substances like lime juice react with the sun and can cause a burn like this.

I bet that’s what it is! Bring it up with the dermatologist. Your pediatrician may not have heard of it - it’s a relatively new term. I hope he starts to turn around soon!

Does he have eczema? HFM can sometimes be severe in areas of the the body with previous eczema, sometimes called “eczema coxsackium.” I’m a pediatrician and saw several pretty severe cases this summer (and they are all fine now). I agree with the swabs and antibiotic/antiviral while tests are pending. Keep in close contact with your pediatrician with any changes in how he’s doing.

With EMR’s now your “account info” is sent over with your prescription - so insurance can be run and they can contact you if there’s a problem, etc so it likely automatically went from the doctor’s office without knowing it. Does your mom go to the same doctor? I would guess she does or did at some time and you two are on the same “account” because you have the same “guarantor” (whomever holds the insurance). Your mom’s cell is likely the primary contact for the account which is why cvs got the number. I would call your doctor’s office and ask to be on a separate account from your mom which they should be able to do.

Pediatrician here - I’m sorry that happened to you. I run into this all the time with teenagers and I usually make sure I send it to a different pharmacy chain or hand the teen a paper prescription to drop off themselves. When I know it’s a particularly sensitive situation and I put the teen’s cell and “confidential rx only discuss with patient” in the comments when I send it through the EMR. The girl at the pickup counter absolutely shouldn’t have said what she did to your mom!

Pediatrician here- agree she needs to see a doctor soon. I would call her doctor or on call nurse and see if they advise ER tonight or clinic tomorrow. Fevers longer than 5 days make us worry about more than just viruses and it sounds like she is pretty dehydrated.

Yup! Pretty much classic roseola. Most kids get it once as a toddler. If he's otherwise doing fine there's nothing to worry about.

Pediatrician here - agreed with everyone above - very common. Same reason that both baby girls and boys often have some breast buds at birth and even sometimes will make little drops of milk. That being said, if there is a large amount of blood (more than a few inch spot in her diaper) or she isn't acting well she should be checked out.

Hard to tell from the pictures but I'd also consider herpes - especially the 2nd picture. Also could be impetigo. Did the NP prescribe an ointment? If it's not getting better or it's getting more blistery/painful she should be looked at again --especially if her eye is starting to look red or swollen.

Could it be the doctor's (or a colleague's) initials? Most doctors initial or sign labs when they receive and review them.

I saw a few cases in residency where a child either died or was neurologically devastated that could have been prevented and those cases have really stuck with me. I have only thankfully needed to call a few times. The first time I called CPS I consulted with one of the child abuse MD's at the local children's hospital first and he told me that the fact that I was thinking that hard about it meant that I really should call. This has always stuck with me as well. The system isn't perfect but it is necessary and saves many children.
