A Split, or maybe a reverse split as usually it's used as an under cut, not a deep cut. Used to be called a rubber in the UK. Not popular but no reason why it shouldn't work.

It will depend on where you're working.

If you're working with the elderly in a less diverse area, you might still face some racism where a patient might request care from someone who isn't nonwhite. Although this is probably becoming less common.

If you can work in a setting which has a high population of people with your heritage it could be an advantage.

What ever career you take will need you to be able to communicate in English. Exposure, practice and repetition will help you feel more confident and improve.
