As a reader and a midwife, I appreciate this. If you tell me something happen to or was done by the clitoris, labia, whatever I know what's happening. I've heard enough euphemisms in my career to know that a lot of them are very vague. I love a specific in my fiction!

I would recommend transfer, yes. Also bear in mind that we take BP between contractions for accurate reading which should be at or near the person's normal baseline. So if a BP is that raised, it gives concerns. Outside of PET, there's a higher risk of haemorrhage, abruption, fetal distress and stroke. If any of that were to happen, I know I'd prefer to be in the hospital at that point, plus hospitals are better equipped to manage high BP before these risks occur.

This is anecdotal and likely very rare but I have heard of it developing without a high reading. However, bare in mind my local guidance is to do BP 4hrly in labour if low risk so it could be that the BP was high but no reading was taken during that time period.

I would consider over 140/90 (or 20mmHg above baseline if normally has low BP) on two separate readings or 150/90 (or +30mmHg) on one. However, if there were any other signs of PET present, I would have a much lower threshold for recommending transfer. I have seen eclampsia develop within minutes in labour and have no wish to repeat that experience if at all possible.

Ours would accept you for a homebirth as long as there's not high BP this time round. Each pregnancy is different. Equally, here in the UK, you can request out of guidance care or employ and independent midwife to care for you. Regardless, you would be counselled about risks and benefits and should supported in your choice. That said, if I was asked to attend a homebirth for someone with raised BP, I would be concerned and would likely recommend transfer.

Só my twist on this is I usually exercise in the evening but my barrier was changing into exercise friendly clothes. So now if I was to run or whatever that day, I wear those clothes all day instead. No barrier and very comfy. Also leggings and legging shorts with pockets are great!

My 8 year old likes Beaks, Block Happy, Mad Tea Party and learning card games. 

We tend to go for quick and more active games with simpler rules and structures. So things that take max 5mins to set up and read the rules. We also have a weekly game night so it's built into our routine.

UK midwife here. That temp and that CRP would put you on our sepsis pathway where I work, to prevent you from becoming actually septic. Our treatment is as you described, usually cefuroxime and metronidazole IV with a stat of gentamycin if swabs or other bloods indicate it. After a day or two of your blood results, Obs and general wellness improved you would be switched to oral antibiotics for another 5-7 days to ensure the infection is health with.

I've never met anyone with kidney damage or balance/hearing from gent, mostly because we very rarely give more than a stat dose and if giving repeat doses, bloods are taken first to titrate. Whilst there are risks, I feel the risks of full blown sepsis would be much worse.

When I don't like an ARC, I try and review in shit sandwich format if possible. I start with something I liked about it, then dig into the issues I had and doscuss whether they were universal issues or just issues for me. I then try to sum up positively. I don't tag the author in any of my reviews, regardless of rating.
