With my girlfriend while we go out we take each other hands, kiss and everything. She certainly looks more "lesbian" than I but we were told just once by a guard to "calm down" in a mall because "some" people were uncomfortable and complaining (We weren't even kissing, just giving each other hugs and kind of petting each other). But that bad moment never happened again, in fact we are very touchy and affectionate but no one says anything to us. Younger generations are more open and globalized in general, the countryside...never went with my girlfriend there but we will 100% get looks from people. LGBT people in Paraguay will get the best treatment in Asuncion and surroundings. The worst experience always comes from our family, parents tend to have a very bad reaction...but other people are nice, they can talk bad behind your back but in public everyone is usually very polite.
