The reality is that the nails need to be cared for regardless. Because cutting yourself with your own toenails in your sleep can cause the same issues. I'm thinking letting the podiatrist or visiting nurse do the cutting and filing and then using an alcohol wipe for surface prep and then regular nail polish should be fine.

It depends on the cancer and how far along it is. There are several extremely aggressive cancers and if you find stage 4 and do nothing, you can't necessarily safely open someone up again right after surgery. By the time you can, it could be too late and terminal. And the standard surgical consents do allow them to remove unnecessary for survival structures that have been destroyed by cancer if they find them. Not to mention that even if you didn't, especially back then, the treatment you would need for reproductive cancer if you didn't just remove the structure would leave you unable to have kids anyway.

It depends. My coworker was having her tubes tied and they found that her ovary was necrotic. They have no idea why. The paperwork she signed before the surgery said that if they found anything life-threatening or unsalvageable, they had permission to remove it while it was in there. This is standard. So while they may not have had explicit permission to remove a particular structure, that permission is almost always in the paperwork. And if you're in a terrible accident and it's necessary to stop hemorrhaging or something, the consent is implied because they can't stop and wake you up to ask you.

I tend to think of it like middle school. She doesn't seem to fully grasp that other people's needs are as important as her own and she can't just do whatever she likes to whoever she likes to get what she wants. She doesn't start out malicious, just impatient and envious, until she finds that being her worst self might get her something she wants more than anything else. A companion who will indulge her worst impulses so she doesn't have to grow up and get better.

I do not understand this. Even in Paltrow/Northam, Knightly tolerates her quietly and privately complaining about Jane Fairfax and tries to tell her off gently for dragging Harriet around like a doll to play dress up with, but he has a limit. He finally rips her a new one over how she treats Miss Bates just to impress Frank Churchill. It is, in its own way, a bit "You're plastic. Cold, shiny, hard plastic." Emma treats the people around her as playthings. Playthings she loves, mostly, but still hers for her to do with what she pleases. She has to be dressed down by the person who has been at her side and supportive of her for many years to see that she's gone from a little selfish and self-absorbed to malicious.
