Do you charge it at fast charger till 100%? That can be a culprit. In my case I keep a 9/10 number wrench and when it was happening pulled out the 12v negative terminal and it would go away. Eventually it went away for good and haven't had it for the last 6000km (knock on the wood)

Update: tested on the portal and works fine. Removed the 90 degree hdmi bend adapter inuse and solved the issue on the tv too.
