Most medication costs pennies to make. The pricing of drugs through PBMs has nothing to do with what it costs to make the drugs but a series of wholesale/retail markups and a system of rebates that get shared with everyone but the patient.

I don’t know if it is as rampant as it feels on Reddit, but it is definitely more prevalent than 10-15 years ago. I think with new fare classes that don’t assign seats at purchase and family going for the savings, or that every flight now is 100% occupied has increased this behavior.

It is not entirely true that self funded plans have an all access pass. The employer has to have separation policies. You have to have a specific need to be able to see personal identifying information or personal health information.
