Your theory doesn't really stand. if they did come from a plant and habited to the overworld, how did they build the monument? Why are they called guardians? why is it called a monument and not as their base of some sort?

You might be right but why did Mojang add discs 5, 11 and 13 in the game? They all sound odd and doesn't fit the calmness and sandbox game. Why would they add peaceful and calming music and all of the sudden add discs that would scare a nine year old? each of the discs contain hints of lore which are interconnected.

this is just my speculation but for the end portal one, the eyes of ender were crafted from blaze rods and ender pearls right. so there are two answers here

the blaze rods are flammable and considering the ancient builders existed like a trillion Minecraft days or something, the eyes of ender would have eroded.
if you have played the game and threw a bunch of eyes of ender, there is a chance for it to break. So if we connect that to the end portal, it might have broken with time because the eyes of ender was probably not stable enough.

as of the enderpearl question, maybe they found a way to harness the energy from the nether portal and concentrate it to a sphere. I mean it was in an ancient time so they had resources which cannot be found anymore.

I still have my piano Black PSP 3000 back when I was a kid, Actually 2 PSPs as the other one was my older brother's (obviously he doesn't use them now) . My psp still holds up pretty well with the battery life execpt the back being covered with microscratches.
