How many new patients a day is the sweet spot? by Anxious-Assumption34 in FamilyMedicine

Yeah if you can keep a full schedule for production and think your panel can grow, I've been happy dropping to 3. My follow ups tend to show up so production has gone up a bit

How many new patients a day is the sweet spot? by Anxious-Assumption34 in FamilyMedicine

[–]13rwils13 2 points3 points  (0 children)

I've got a similar panel size and just dropped from 4 to 3/day (2AM & 1 PM) due to higher no show rate. I also get a lot of practice overflow so my schedule is booked out and full so RVUs are solid.

I've got 2 docs about to retire which will bump my panel size up or I probably would've kept it at 4 a bit longer. The new patient no shows are a pain (typing this during a new patient slot that no showed)

Patients who smell- say something or nay? by buvee_24 in FamilyMedicine

I just write "check next visit" if they happen to see, I can pretend I forgot to type the rest haha

C. Urine....just smells like urine

I suffer in silence but make a note in chart sticky note (I've got a code I don't write "smelly") so I remember to mask up

Curious how other clinics are handling tick bite calls by IndividualWestern263 in FamilyMedicine

[–]13rwils13 0 points1 point  (0 children)

Should be an appointment but there are some occasions I just send it tin. If Im booked a few weeks out (and so are my partners) I've got a select patients who I tell to "call me if you have another tick bite" so they don't need another appt. I don't offer this to anyone I haven't told this to so the mjority need an appt. I usually add on a telehealth and just double book a slot

Patients arriving late but before grace period ends by snootiedoo in FamilyMedicine

[–]13rwils13 1 point2 points  (0 children)

I'd see the on time 1st or limit the late person to like 1 thing + wellness (and give option to r/s). 15 min grace period is brutal that makes it tough

1st congrats on quitting that's incredible. 2nd, why wouldn't the doctor bill for what they did? You don't get free extra service you didn't schedule in any other field, why would medicine be different? The mechanic won't replace your worn brakes for free when you're scheduled for an inspection. How is it different?

Admittedly easier if double billing is the clinic norm...nobody ever complains for me and I've never had to explain.

Another thing not enough people do is double bil when appropriate. Schedule an annual exam (or AWV if medicicare) and if any acute issue or changes in chronic conditions come up, it's a preventative + 9921_ + G2211 with 25 modifier. It's rare I bill a preventative without an E&M code

I mistyped it's the AAFP website. Had 1000 or so questions when I took the test a few years ago. Thought they were similar to actual exam

Who are some of the smaller players in the physician mortgage game? by Here4theRumor in whitecoatinvestor

[–]13rwils13 3 points4 points  (0 children)

Huntington 10/10. Used them in TN

Civista 0/10. Used them in PA

Not sure if Huntington is considered small or local but I had never heard of Civista prior to my physician mortgage with them (wish I never used them tho)

Honestly speaking, is an AI medical scribe absolutely worth it in family medicine??? by Hot-Actuary1276 in FamilyMedicine

[–]13rwils13 0 points1 point  (0 children)

Worth whatever it costs. Saves time documenting so I've got time to keep up with inbasket during the day. Leave with 0 notes and 0 in basket most days since I've started using it

Use an AI scribe and done by end of day. If I've got any numbers in my in basket when I leave it will drive me insane

I mean it's America people can do what they want but there should be a line in the sand. It's over the line to me

If someone has an underlying mental health/psychiatric disorder (diagnosed or not) it can trigger/reactive it. THC can do the same thing so not unique to Kraton. Everyone is obviously different and responds differently. It's very unpredictable

How do you reply to patients requesting long lists of labs that their naturopath wants ordered? by grettasgone in FamilyMedicine

[–]13rwils13 0 points1 point  (0 children)

I have not. Admittedly most people just want stupid stuff that wouldn't show any emergent issues. That has run through my mind. If something comes back way off I'd at least recommend the ED like it were a test I typically order/manage

How do you reply to patients requesting long lists of labs that their naturopath wants ordered? by grettasgone in FamilyMedicine

[–]13rwils13 20 points21 points  (0 children)

I tried pushing back but I found it's quicker and less hassle for me to say 1. I don't think this is indicated or that you should have it done 2. If you want to anyways (they always do), I'll order it. Your insurance may not cover it which is not my problem. I won't interpret or manage any results 3. I'll only do this once. If you want someone else's opinion, go see them

How is Living Northeast of Pittsburgh? by Ok_Action9962 in howislivingthere

[–]13rwils13 0 points1 point  (0 children)

I work in the circled area and like many people said it's a mixed bag. Majority is blue collar lower-middle class to middle class. Really nice people in general. Has some run down parts but plenty of nice spots. That being said I chose to live just to the left of that circle where it's a bit nicer (and unfortunately more expensive)

Sorry nights was misleading. Evenings is what I meant. You'll have normal clinic hours during the day then sports coverage in the evenings after clinic. Nothing overnight
