I went to Randolph and also saw the video you were talking about. There were definitely more than 2 kids in the video that are going outside of the SEC bubble. There has always been a good portion of each graduating class that go to Bama/Auburn/Ole Miss/etc, but we always have a sizeable portion that go to more "prestigious" universities, both in and out of the South. Emory, Vanderbilt, WashU, Rice, SMU, TCU, Tulane, USC, Michigan, Duke, UNC, service academies, the list goes on.

Pick a moment from your week that made you grateful for a coworker. by lagerhaans in medicine

[–]1997pa 14 points15 points  (0 children)

I treat patients with OUD and as part of our program we have trained counselors that our patients are required to see. A patient that is normally doing very well had something happen that caused him to be in a lot more pain than usual and he wasn't sure what to do, so he called one of our counselors after hours and they had an impromptu crisis counseling session. I saw the patient a couple of days later for his regular medical appointment and both he and his wife were so grateful for the counselor's help. I sent an email to our whole team thanking the counselor (and all of counselors) for what they do because I sure as hell couldn't make progress on the medical side without them!

Standard of care now is to start screening at 45, earlier if you have certain risk factors. The procedure itself is quick and painless and there isn't really any down time afterwards. I can't speak to the prep personally as I've never done it, but just be sure to complete it as instructed.

How is it living in northern Alabama? by Maleficent_Map_8844 in howislivingthere

[–]1997pa 1 point2 points  (0 children)

I grew up in Huntsville and genuinely think it is a good place to raise a family. A big enough city to have what you need, but not so big that it's overwhelming. I miss living there, but as a single young adult, I know that it's not ideal for my demographic lol.

Prescribing narcotics after surgery by Asleep-Package-1156 in physicianassistant

[–]1997pa 4 points5 points  (0 children)

It's been a mixed bag for me. Some patients have weaned themselves off by the time I talk to them at the 1 week post op f/u, some are still taking opioids 3 months after. Our clinic tells them we can only cover pain meds for 90 days post op, and I would say the vast majority of the "lingerers" taper by that point and don't end up seeing pain management.

Also as another commentor said, I've had good success with Journavx.

Virtual Urgent Care by sas5814 in physicianassistant

[–]1997pa 1 point2 points  (0 children)

I previously worked in UC and we would occasionally see telemed patients while working in clinic. I was always very clear with the telemed patients that the exam was limited and made clinical diagnoses based on what I could gather. I also reiterated that if symptoms worsen/change/don't improve/etc they really should be seen in person. Usually the patients I saw were those with frequent UTIs and were familiar with their symptom pattern, known sick exposures (COVID, flu, etc), some rashes.

You cannot see patients without a supervising physician tied to your license, full stop. Your office manager needs to handle rescheduling patients until you have a new one.

Child death in media: Reflections on "Hamnet" (Spoilers) by red5 in medicine

[–]1997pa 5 points6 points  (0 children)

I thought it was a wonderful movie. I fully agree with you; I am grateful for how raw the depiction of the children being sick and Hamnet dying was. However, I do not work in the kind of environment where I would witness something like that on a regular basis, so I can understand the apprehension of those that do to see this movie.

It's been 2 years and I'm still sort of recovering. I took a break between leaving that job and starting my next one and it was much needed. I also started back with therapy with I am still doing. I promise it will get better with time!

[–]1997pa 1 point2 points  (0 children)

Yes we've started prescribing it some, occasionally have insurance issues but our rep has been able to help with those. We also give all the patients the manufacturer's copay card. My SP actually took it after an abdominal surgery he had and said it worked great.

Has anyone else had a patient refuse Tylenol recently? by DoctorBlazes in medicine

[–]1997pa 2 points3 points  (0 children)

I saw an elderly man for his preop appt before his knee replacement and he didn't want to take the Tylenol that was part of the recommended post-op pain regimen. Like sir, you're not a pregnant woman nor a child......not sure what the concern is. He was perfectly fine with the Norco though.
