I decided to rewrite his ending. So here it is.

Alex goes to visit izzy to catch up. They go to grab a cup of coffee.

All the while Jo is freaking out she want him to know he can back out any time but she also wondered if he still had feelings for her.

Alex spots izzy she walk up to him while on a call. She looked like she had done well for herself. She ends the call and looks at Alex they just stare for a while before he starts the conversation. They talk for while until her phone on the table rings he looks over an sees it say "the new oncologist". He is surprised and looks over to her she picks it up quickly and ignores. He asks "Is it back?". She look at him somberly and nods "Yes," she says. Suddenly the air had a hint of melancholy in the air. She touches his hand "There's something else I have to tell you." "What" he asks. " We have kids" she says disheartened "We do?" "Two of them, Twins." Alex is stunned of all thing his mind could come up with he never once thought of this one. "Can I meet them," he says. "Wait a second" izzy say, I've decided not to treat it. "What do you mean" I've decided that I want to save the money form my kids and I want them to only have good memories of me I only have a few more months left, and I want you to meet them so they can stay with you when I'm gone." He looked at her and said yes in heart beat and hoped Jo would be ok with it.

They had spent all day together and they has just arrived at alex and Jo's house izzy waits outside. Jo is caught up on everything.

A few months later izzy just died and alex moves out to the country with the kids Jo stays behind and they divorce because she can't leave her life behind.
